Nutrition Counseling ICD-10 Codes for Dietitians

What is an ICD-10 Code?

An ICD-10 code is a type of medical code used to represent diagnoses, symptoms and procedures. Instead of writing “diabetes” on an insurance claim form, a corresponding ICD-10 code, such as E11.9, is used instead.

Some examples of ICD-10 codes with their corresponding descriptions include:

  • E11.9 -Type 2 diabetes mellitus without complications
  • R51.9 – Headache, unspecified
  • Z00.00 – Encounter for general adult adult medical examination without abnormal findings

ICD-10 stands for International Classification of Diseases, 10th edition. It was designed by the World Health Organization (WHO) and then adapted for the United States by the Centers for Medicare & Medicaid Services (CMS) and the National Center for Health Statistics (NCHS). 

Why are ICD-10 Codes Used?

When it comes to insurance billing, codes create a universal language which makes the billing process simpler and more standardized. Just like CPT codes are used to describe the medical services provided, ICD-10 codes are used to describe the diagnoses treated.

In addition to streamlining the billing process, ICD-10 codes are also used to collect data for quality measurement, resource utilization, and monitoring outcomes. 

Where do ICD-10 Codes Go?

The ICD-10 code(s) go in box 21 on the CMS-1500 form. Note that you should not include the decimal point of the ICD-10 code.

How Many ICD-10 Codes Can Be Listed?

Box 21 on the CMS-1500 form has space for diagnosis codes A through L, so up to 12 codes can be listed. However, it is unnecessary for dietitians to list this many codes. For most claims, one to two ICD-10 codes will be needed.

Can Dietitians Use ICD-10 Codes?

Dietitians need to use ICD-10 codes to bill insurance companies for nutrition counseling services. If a claim does not have any ICD-10 code(s) listed, it will be denied. 

However, diagnosing medical conditions is not in a dietitian’s scope of practice, so using ICD-10 codes has to be done carefully. 

Documentation of Medical Codes

Dietitians are able to use medical codes (e.g. diabetes, hyperlipidemia, hypertension) for billing.  For example, if you are providing medical nutrition therapy for diabetes, using a diabetes diagnosis would be the best fit. 

However, since dietitians are unable to diagnose this medical condition, this code will have to be documented by a different healthcare provider that can diagnose, such as a physician. Therefore, either medical records or a referral with the corresponding ICD-10 code will need to be collected in order to use it.

Even if the patient tells you they have diabetes, and you see paperwork from their physician stating they have diabetes, this is not enough. The actual code needs to be documented somewhere. If you simply ask the doctor’s office for a most recent progress note,  the diagnosis codes will most likely be on there, as most electronic health records automatically include ICD-10 codes next to diagnoses. 

Once in a while, at my office, we are unable to get written documentation of the ICD-10 code we need to bill with. In this case, we call the provider office to get information over the phone. We then document the verbal information provided in the patient’s chart along with the name of the staff member. Best practice is to get written documentation rather than verbal.

Z Codes

Z codes are described as “factors influencing health status and contact with health services.” Since these are not diagnostic codes, dietitians can use these without documentation. Some examples of Z codes include:

  • Z71.3 – Dietary counseling and surveillance
  • Z68.30 – BMI 30.0-30.9, adult
  • Z59.41 – Food insecurity

ICD-10 Codes Related to Dietary Counseling

There are thousands and thousands of ICD-10 codes, many of which can apply to dietitians. Below you can find several lists of ICD-10 codes dietitians are using. Note that these lists are not exhaustive. 

Z71.3

Dietary counseling and surveillance is code Z71.3 and is often used by dietitians, for a variety of reasons. This code is appropriate if a dietitian is meeting with something in which there is no specific diagnosis. So if the patient is a healthy individual, this code is fair game. Also, for some insurance companies to cover nutrition counseling as a preventive visit, Z71.3 must be the first and sometimes only code used.
